- [F1]On January 29, 2004, Mark Wyckoff filed a Form 4 reflecting the vesting of certain shares of contingent stock on January 29, 2004. These shares had already been reported on a Form 5 filed on December 31, 2000 as part of a grant of contingent stock on January 29, 2000 and January 1, 2001. This resulted in an overstatement of total shares held by Mr. Wyckoff. The January 29, 2004 Form 4 should have reported not the vesting of the shares, but that certain of the shares originally granted did not vest.
