- [F2]On or about 9/30/2003, Quick-Med Technologies, Inc. and Mr. Granito agreed to consolidate two existing Notes, one of which was a Convertible Note. As a result of the consolidation, the Conversion price changed from $1.00 per common share to $0.38 per common share. Additionally, on or about 08/4/2004, Quick-Med Technologies, Inc. and Mr. Granito agreed to extend the maturity date of the Convertible Note from 7/1/2004 to 7/31/2005. On or about 9/30/2004, Quick-Med Technologies, Inc. and Mr. Granito agreed to extend the maturity date of the note from 7/31/2005 to 7/1/2006. As a result of the 7/30/2004 amendment, 7/1/2006 is the curent maturity date of the Converitible Note. On 11/30/2004, $500,000 of indebtedness was converted into 1,315,790 shares of common stock at $0.38 per share. In addition, on 12/31/2004, $826,087 of indebtedness was converted into 2,173,913 shares of common stock at conversion rate of $0.38 per share.
